Ordering Stationery
Trinity Terminal
To order stationery for your Trinity terminal please complete and submit the attached form.


VX Terminal
If you are using a VX terminal you can order stationery directly from your device (press 
FUNCTION followed by 7 to access this menu).
Add Providers to your Terminal
To add a Provider to an existing HICAPS facility please click here and complete the "Register a new Provider" form.

Adding Providers generally takes between 2 to 3 business days to complete.

To avoid delays, please be sure to provide all required documents and ensure that responses provided match those within your Medicare / Medibank Provider Letter.
